Jewish Family Service of MetroWest New Jersey

The preference to age at home rather than in a nursing facility is a growing trend among the older adult population. Jewish Family Service of MetroWest offers case management services for Holocaust Survivors allowing them to continue to live in their own homes for as long as possible. Grant funds will support 120 hours of case management services through social work assessments, treatment plans and referrals to a network of home health aides, medical professionals, clergy and friendly visiting services. Financial, physical and social needs are also addressed and continually monitored through case management services with the survivor and their family.
