Capital Caring

On days when hospice visits are not scheduled in the home, the “TeleCaring” program – a medical hotline – is accessible for the “Capital Caring” health care team to reach out to patietns and families. Case managers and specially trained nurses check in each morning and evening to determine the patient’s and caregiver’s well-being, pain level and symptom management. This grant supports the expansion from a pilot project of one Capital Caring service areas to all seven Capital Caring regions.
